• At Mt. Lookout Dentistry, we believe treatment decisions should be based on what is best for your health—not on the limitations of an insurance plan. As an unrestricted (out-of-network) provider with all dental insurance companies, we maintain the freedom to provide personalized care without being influenced by insurance company restrictions.
  
  As a courtesy to our patients, we will gladly submit pre-treatment estimates (pre-determinations) to your dental insurance company whenever appropriate. This allows you to better understand your anticipated insurance benefits and estimated out-of-pocket costs before treatment begins.
  
  We are also happy to submit your dental claims on your behalf. Depending on your insurance plan, reimbursement may be sent directly to you by your insurance company. Because benefit payments vary by plan, payment for services is due at the time of your appointment.
  
  For patients without dental insurance, we offer our in-house Membership Plan. The plan includes preventive care and exclusive discounts on additional treatment, with no deductibles, waiting periods, annual maximums, or claim forms.
